xcf128x :Failed at address, 0.

I use XCF128X on my board to support virtex6 FPGA XC6VLX240T, but I cannot program this flash probably.

I read ug438 and check my schematic to the correct pin connections in the data sheet. I use slave selectmap mode .
the m[2:0] = 110 and the frequency of cclk is 40MHz

I use iMPACT 11.5 and the log is attached as following:

Boundary-scan chain validated successfully.
1: Device Temperature: Current Reading:   36.10 C, Min. Reading:   30.20 C, Max. Reading:   36.59 C

Run standalone Erase, Program and Verify, what do you get?
If you only fail in the Verify process, try to readback the content of 128xl and see if the image you programmed into it is correct.
